Position Description:In coordination with the Director of Marketing and Business Development, creates a plan to identify, qualify and build relationships with key referral sources from Long Term Care (LTC) facilities, Assisted Living (AL) facilities, physician practices (Internal Medicine, Family Practice and other specialties) in a given territory to grow Kansas City Hospice & Palliative Care’s (KCH&PC’s) business. Collaborates closely with the Hospital Clinical Liaison to facilitate patient transition into Long Term Care.
