</div><div><strong>Responsibilities:</strong></div><ul>
<li>Conduct comprehensive patient assessments and develop individualized care plans.</li>
<li>Coordinate admissions and ensure a smooth transition for new patients.</li>
<li>Educate patients and families on care procedures and health management.</li>
<li>Collaborate with interdisciplinary teams to optimize patient outcomes.</li>
<li>Monitor patient progress and adjust care plans as necessary.</li>
<li>Maintain accurate and up-to-date patient records and documentation.</li>
<li>Participate in quality improvement initiatives and patient safety programs.</li>
<li>Provide emotional support and guidance to patients and their families.</li>
</ul><div><strong>Requirements:</strong></div><ul>
<li>Current RN license in the state of California.</li>
<li>Minimum of 2 years of nursing experience, preferably in home health or admissions.</li>
<li>Strong assessment and critical thinking skills.</li>
<li>Excellent communication and interpersonal skills.</li>
<li>Ability to work independently and manage time effectively.</li>
<li>Basic Life Support (BLS) certification required.</li>
<li>Experience with electronic health records (EHR) preferred.</li>
<li>Compassionate demeanor with a patient-centered approach.</li>
