<br>Owner Operator Requirements:</div><ul>
<li>Copy of your driver license</li>
<li>Copy of your social security card</li>
<li>Proof of Unladen Liability Insurance of $ 1,000,000</li>
<li>DOT truck inspection</li>
<li>DOT Physical (long form AND expiration card)</li>
<li>Drug test to be taken in Elkhart, Indiana</li>
<li>Copy of truck registration - must have 26,000 pounds (no more) listed on registration.</li>
<li>Power of Attorney form (if truck is not owned by the driver)</li>
<li>Personal DMV report (5 year driving history)</li>
<li>Must have a smart cell phone with email capability</li>
<li>Age Requirement: 25 without a CDL or 23 with a CDL</li>
<li>Experience Requirement: Minimum of 2 years documented hauling experience</li>
<li>No accidents or speeding tickets within the last 3 years</li>
<li>No more than 1 DOT violation within the past 12 months</li>
</ul><div>Truck Requirements:</div><ul>
<li>2016 model year or newer, 1 ton or 3/4 ton diesel truck</li>
<li>Fifth wheel hitch (16,000 to 20,000 lbs. rating recommended)</li>
<li>Travel trailer weight distribution hitch with 2 5/16” ball and sway bars</li>
<li>12-Volt marine battery with studs and wing nuts and battery box</li>
<li>Full width mud flaps (4” above the ground when loaded)</li>
<li>26,000 pound rated plate</li>
<li>Safety kit (bracket mounted fire extinguisher and 3 triangles)</li>
<li>Hand tools, 4-way tire wrench, 5 ton jack</li>
<li>Nylon nut-bolt sets for transporter plates</li>
<li>Padlock or pin suitable for latching hitch coupler</li>
<li>No radar detector allowed in truck</li>
<li>No window tint on driver and front passenger window. Windows must allow 70% of light</li>
<li>Dash Cam</li>
