Nurse Practitioner, Clinical Lead, Remote Therapeutic Monitoring (RTM) and Behavioral Health Integra

We are hiring a California-licensed Nurse Practitioner to serve as the Clinical Lead for a Medicare-covered Remote Therapeutic Monitoring (RTM) and Behavioral Health Integration (BHI) program. This is a senior clinical role. You will own patient intake, care plan authorship, standing order development, and clinical supervision of a growing team of RNs and LPNs delivering longitudinal monthly engagement to enrolled Medicare beneficiaries.
You will operate under the general supervision of the Medical Director, with meaningful autonomy within physician-approved standing orders.
- What You'll Do
- Perform intake and clinical assessment. Conduct structured intake assessments for new enrollees, confirm therapeutic appropriateness, and establish baseline clinical status across RTM and BHI domains.
- Author care plans. Create individualized RTM and BHI care plans, assign ICD-10 codes, and document medical necessity to Medicare standard. Revise care plans in response to PROM trends, validated symptom scales, and patient-reported changes.
- Develop and maintain standing orders. Build and iterate the standing order library that governs deterministic protocol execution by RNs and LPNs. Submit standing orders to the Medical Director for approval.
- Supervise clinical staff. Provide day-to-day clinical oversight of RN and LPN care managers. Review their documentation, coach their patient interactions, and ensure compliance with general supervision standards.
- Manage escalations. Own yellow-tier and red-tier escalation review. Make clinical judgment calls on when to intensify, pause, or route patients to higher-acuity care. Coordinate with the Medical Director on complex cases.
- Own behavioral health care. Author and revise behavioral health care plans. Review validated scale trends (PHQ-9, GAD-7, PEG-3, and similar). Ensure BHI documentation satisfies CMS requirements for CPT 99484.
- Support billing integrity. Partner with the compliance team to ensure that RTM (98975, 98977, 98980, 98981, 98985) and BHI (99484) documentation meets time, interactive communication, and medical necessity thresholds before claim submission.
- Required Qualifications
- Active, unencumbered California Nurse Practitioner license in good standing
- National certification (AANP or ANCC) as FNP, AGPCNP, or PMHNP
- California Furnishing Number (active)
- Medicare-enrolled (855I), with willingness to reassign benefits to the employing practice via 855R
- Minimum 3 years of clinical NP experience
- Direct experience with Medicare care management programs (RTM, RPM, CCM, BHI, PCM, or Collaborative Care)
- Prior experience supervising RN or LPN clinical staff
- Fluency with standing orders, protocol-based practice, and general supervision frameworks
- Strong documentation discipline and comfort navigating modern EHRs and digital health platforms
