  • Document Verification and Authentication: Thoroughly review forms for authentication purposes, including verification of member information such as First Name (FN), Last Name (LN), Date of Birth (DOB), identification numbers, residential addresses, and validity checks within our CDB (Central Database) system.

  • Compliance Verification: Ensure all document requirements are fully completed, including proper signatures and dates on all forms before processing.

  • Clinical Data Entry: Accurately enter necessary information into clinical applications, maintaining the highest standards of precision and attention to detail.

  • Form Fulfillment: Process and send forms to members through the FOS system, including selecting the appropriate kit or package based on member requirements and eligibility.

  • Data Tracking and Management: Track received data systematically, ensuring proper documentation and follow-up procedures are maintained.

  • Data Sorting and Interpretation: Sort, interpret, and verify incoming data to ensure accuracy and consistency across all databases.

  • Issue Resolution: Resolve questions or inconsistencies with source documents, escalating complex issues as appropriate while maintaining data integrity.

  • Inquiry Response: Respond to inquiries regarding entered data, providing professional and accurate information to internal and external stakeholders as needed.
