Key Responsibilities

- Family Empowerment and Support: Work collaboratively with families who have achieved permanency to identify their unique strengths, challenges, and needs. Develop individualized support plans that address these needs and promote family stability.
- Resource Connection: Serve as a bridge between families and community resources, including healthcare services, educational programs, housing assistance, counseling services, financial support programs, and recreational opportunities. Help families navigate these systems effectively.
- Advocacy: Advocate on behalf of children and families within various systems, including schools, healthcare providers, social services, and community organizations. Ensure that families' voices are heard and their needs are met.
- Progress Monitoring: Regularly assess family progress toward goals outlined in support plans. Document outcomes and adjust interventions as needed to ensure continued progress.
