Job Title: Geriatric Skilled Nursing (LPN/ RN)

Position Summary


Altra Home Health is seeking compassionate and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide high-quality, patient-centered care in the home setting. In this role, you will care for both pediatric and adult patients, many of whom have complex medical needs such as gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and, at times, require tracheostomy or ventilator support. You will play a vital role in promoting safety, independence, and quality of life while ensuring all care is delivered in accordance with physician orders, individualized care plans, and Indiana Medicaid requirements.


Key Responsibilities




  • Perform comprehensive head-to-toe assessments and implement individualized plans of care.




  • Provide skilled nursing services, including:




    • Medication administration.




    • Wound care.




    • G-tube feedings.




    • Tracheostomy and ventilator care (if applicable).




    • Chronic disease management.






  • Deliver compassionate, professional care tailored to each patient’s needs.




  • Educate patients and caregivers on disease processes, medications, emergency procedures, and self-care techniques.




  • Collaborate with physicians, therapists, case managers, and interdisciplinary team members.




  • Ensure all services are medically necessary and properly documented to meet Medicaid and agency standards.




  • Maintain accurate, timely documentation in the electronic health record (EHR).




  • Participate in care conferences, quality assurance activities, and audits as needed.




  • Uphold infection control, safety, and patient privacy standards at all times.




  • Advocate for patient needs and assist with accessing community resources when appropriate.




Shift Information




  • Shift lengths range from 4 to 12 hours based on patient needs.




  • Flexible scheduling available, including days, evenings, nights, and weekends.




  • Opportunities for consistent, long-term cases or flexible PRN scheduling.




Qualifications


For LPNs:




  • Active Licensed Practical Nurse (LPN) license in Indiana or a compact state.




For RNs:




  • Current and unrestricted Registered Nurse (RN) license in Indiana.




For All Candidates




  • Minimum of 1 year of nursing experience (home health and/or pediatric experience preferred).




  • Experience with G-tube, trach, or ventilator care preferred (training available).




  • Knowledge of Indiana Medicaid guidelines and documentation standards preferred (especially for RNs).




  • Current CPR certification.




  • Strong clinical judgment and critical thinking skills.




  • Excellent communication and interpersonal abilities.




  • Ability to work independently in a home setting.




  • Valid driver’s license and reliable transportation.




  • Experience with electronic documentation systems preferred.
