We are seeking a dedicated and compassionate Hospice Social Worker to join our team at Hospice of Western & Central Massachusetts in Worcester County. As a part-time Social Worker, you will travel to patients' homes and provide essential social services to meet their emotional and social needs. This is a unique opportunity to make a meaningful difference in the lives of patients and their families.
This is an on-the-road position that requires the candidate to travel and have a reliable and insured vehicle. Weekly pay ranging from $41 to $44 an hour. LICSW or LCSW required. 24 hours a week.
Responsibilities:
Assess the psychosocial status of patients and families/caregivers and communicate findings to the interdisciplinary team
Carry out social evaluations and plan interventions based on evaluation findings
Counsel patients and family/caregivers as needed and provide crisis intervention when necessary
Educate patients and families on advanced directives and practical needs
Provide information and referral services for patients and families/caregivers
Serve as a liaison between patients and community agencies
Maintain clinical records and participate in quality assessment performance improvement teams
Requirements:
Master's in Social Work required
Massachusetts LICSW or LCSW required
Experience in healthcare and hospice preferred
Ability to read, write, speak, and understand English
Must be a licensed driver with an insured automobile
Possess the ability to make independent decisions and deal tactfully with personnel, residents, family members, and the general public
